Hi,
Might be of interest to know that:
? on Mac OS X 10.3.6 with Adobe Reader 6.0.2 (stand alone), it's fine, 
including form functionality.
? on the same platform with Aper?u (Viewer?) the type is OK but the 
check boxes don't show up at all.
? on Mac OS 9.2 with Acrobat Reader 4.0 it's fine, with form functionality.
