Window manager has nothing to do with toolbars. Its job is to draw
windows and their decorations.

Ideally desktop environment should set preferred icon size for
consistence sake, then apppications would be able to override that
(even though I personally wouldn't recommend that) via APi available
in toolkit/framework (Qt).

Agreed. However some applications have custom solutions for that
(GIMP, Inkscape, LibreOffice etc.).

There's nothing special about 12920xwhatever (running 1920x1200 here).
It's not even largest resolution available on consumer market (check
recent iMacs).

The team never found a designer to bring some consistence to existing icon set,
let alone drawing new bigger ones :)
