Thanks for the suggestion. This one is too restrictive. However, as I mentioned in an earlier post, I think I have been trying to make life too complicated for myself.  I don't think I actually need to use any license, just say what can be copied and under what conditions 
without the complications of doing it under some license.

particularly at the small box at the bottom-center of the first page. Might a
