Can you not just have in there the QTDIR=... bit?

That's fine - there will be many headers in there.

If libqt is in /sw/lib then the lib directory will be that, however, Qt
tends to keep everything in it's own directory so upgrading is simpler.

Pass. Have you asked on the Qt-interest mailing list which is run by
Trolltech? I'm working off a limited knowledge of how OSX sets things up
here.

Linux is dead easy, pass on why fink should be difficult though.
