It also depends whether the book will be paperback or hardcover. For
paperback take the internal page width, multiply by two, add the estimated
spine size, and, if needed add a bleed (3-5 mm on each side). I usually add
another 6 mm on each side for placing registration marks (showing where the
edges are, but also the spine). Add these together and get the document
width. Take the page height, add the bleeds, and space for registration
marks, and you have the document height.

The printing press will tell you about what bleeds you need, and whether you
need registration marks. They might also want cmyk boxes on the edges. I
reuse the boxes from previous book covers that I have made. I also have the
words "Cyan", "Magenta", etc. written in those colours on the edge. This is
for separating colours (an option when printing to postscript.
Unfortunately, you will probably have to edit the postscript file manually
to adjust the frequency and angle...)

Hardcovers need more space (generally 2cm) for folding and because the size
is larger than the inner page.
